A.I.R.E. REGISTRATION & UPDATES
For A.I.R.E. Registration updates, forms can be downloaded from the Chicago web page (under "Forms"), and must accompany your passport request, along with the following REQUIRED DOCUMENTATION AS APPLICABLE:
  • A “Certified Copy” of the “Marriage Application”, the “Marriage License” and the “Marriage Certificate”  from the County Court in which they were issued and registered;

  • A "Certified Copy" of the “Divorce Decrees” and the accompanying “Statement of No Appeal”. Both of these documents must be translated into Italian;

  • "Certified Copy" of applicable Colorado birth and death certificates. These can be obtained from the Colorado Department of Health, Vital Records or Vital Statistics. For access to documents that were issued out of state, call this office at 303-224-9927 for information.

    NOTE: Each of these documents must have an “Apostille” attached to them!

    For an Apostille, send all documents (or present them in person) to the Secretary of State of the state in which the document was issued. The “Apostille” verifies the signature of the official who signed the Certified Copy. In Colorado the fee is $2.00 per document for mail-return and next day service OR an additional $15 per document for document processing while you wait (payable in cash or money order only). Mail service requests should include a cover letter indicating the country of destination for the Apostille and a self-addressed stamped envelope
