STEP BY STEP APPLICATION INSTRUCTIONS

then SCHEDULE AN APPOINTMENT with my office and sign them before me.

IMPORTANT DOCUMENTS TO BRING WITH YOU

  • U.S. or international passport/travel document that is valid six months after departure date from Italy . If you are not a U.S. citizen please bring your Green Card, PLUS COPY both front and back, or proof of your U.S. Student Visa, COPY ONLY whether it is in your passport or a separate document that you acquired from the USCIS.

  • One current color passport photo which you can obtain at any Kinko’s, Walgreen Drugs, or T riple A,
    (AAA T ravel). Please make sure that it is
    different from the photo that is in your passport.

  • a copy of your Drivers License which will be notarized at this Office. It does not have to be Colorado License because being a student in the state means you are a temporary “resident”.

  • A copy of your Student I.D, which will be notarized as well.

  • Original, PLUS COPY of your current enrollment at a U.S. University. What the Consulate needs to see is that you will be returning the following semester to continue towards graduate or post-graduate studies. If you cannot obtain one from the exchange program you may acquire it at your school’s Registrar’s office.

  • Original PLUS COPY of your letter of enrollment at the school in Italy . Many schools provide you with two copies, one it Italian, one in English. Make sure you include a COPY of each.

 
Affidavit of Support - Requirements

  • Must be signed by the person who will be covering your expenses while you’re in Italy . If it’s a parent and they cannot come to my Office then they must have their signature notarized either at their local bank or UPS Store. Utilize the site listed above. *

  • Proof of “Financial Means”. Whether it is you, your parents, grandparents or other family members, their most recent bank statement, be it savings, checking, Money Market, etc. PLUS COPY, or two copies of the original, must be presented. T he rule is that there should be approximately $1,000.00 for every month that you are there. If you have arranged for Financial Loans through your school, you must provide a breakdown PLUS COPY of such loans. Banking on-line information is acceptable as long as there is a name that corresponds with whomever is signing the “Affidavit of Support”. If you are uncomfortable in submitting this information, please know that this remains strictly confidential, do not include any documents that include your SS number and you are certainly  welcome to “black out” your account number. A letter from the bank also works as long as a balance is indicated.

Utilize the site listed above. * T his will also be notarized by this Office.

  • Flight Itinerary, PLUS COPY of your “round trip flight”. Your visa will only reflect the dates of your study program. If you plan on touring after your program is over, remember that you revert back to a “tourist” and can stay a while longer, however Italy needs to know that you are indeed returning to the States. If you do not have a return flight than your letter of enrollment from the U.S. school should indicate that you are continuing your studies at the U.S.

Note: This office will greatly appreciate your complete Visa application packet at the time of your visit; however, if you are missing a few items at that time, don't panic. We will review and YOU WILL INCLUDE them in your packet before you mail it to Chicago .
