TOURIST VISA REQUIREMENTS

·         U.S. or international passport/travel document that is valid ninety days after departure date from Italy PLUS COPY of page with picture only. If you are not a U.S. citizen please bring your Green Card, PLUS COPY both front and back or your Visa COPY ONLY whether it is in your passport or a separate document that you acquired from USCIS.

·         Completed Schengen Application Form.

·         One recent color passport photo.

·         Copy of your Colorado or Wyoming Driver’s License. If not available please include photocopy of your utility bill that proves your residency in these states.

·         Letter from your employer on company letterhead and original signature indicating length of employment, current salary and your travel dates PLUS COPY. If applicant is unemployed this letter still needs to be produced by the employed spouse as well as a copy of your Marriage Certificate. 

·         Most recent bank statement that should show $1, 000.00 for every month you are there PLUS COPY. Only the first page is needed as long as it includes your name and the name of the institution. Please black out you account number and DO NOT include any documents that list your Social Security number!

·         Flight Itinerary, PLUS COPY.

·         Hotel lodging PLUS COPY.

·         If staying with friends or family you must procure a “Lettera Di Invito” from YOUR HOST in Italy ; their original signatures must be on these documents which are downloadable from the Chicago website. You will have to arrange for the mailing processes because only hard copies are excepted, no faxes, photocopies or scans. Please make sure they enclose a copy of their ID.

·         Note that there are two “Lettera Di Invito forms, one for family, “Familiare” and another for tourism or  “Turismo”. Please utilize the one that applies to you.

·         Copy of your health insurance policy pages that state “International Coverage”, “Overseas Coverage” or “Out of System Coverage”, etc. You must also include wording from this policy that states a minimum of $50,000.00 coverage.

·         Fee payable in Money Order or Certified Check ONLY. No personal checks or cash.

  • Follow all the instructions carefully and completely;
  • Click on "Visa Application Form" to download one

You must complete both forms accurately and completely, then come to my office and sign them before me.

If you need copies to be made, this office as well as Chicago's Consulate office will assess a $5.00 charge payable in cash or check.

Express mail envelopes and pre-printed mailing labels are provided free of charge as needed to submit your application.
